Provider Information

• When you see a physician practice listed as a participating provider in the Inclusive Health network, remember that not every physician in that practice may be credentialed and considered an in-network provider. If you select a physician that is not an Inclusive Health participating provider, your benefits may be reduced.
• When you arrive at a physician’s office, hospital, or facility, present your health insurance card bearing the Inclusive Health logo so the provider registers you as an Inclusive Health participant. Before making an appointment with a physician, you should confirm he or she is still a participating Inclusive Health provider.
