Inclusive Health Assist
Monthly Premium Financial Assistance

This federally funded program provides discounts of 20% - 43% off of monthly Inclusive Health premiums for those who qualify. The discount is only for the Inclusive Health member or potential member who qualifies for the State Option. The qualifications are based on an individual's household size and income.

Inclusive Health is accepting applications for 2013 for the IH Assist and Premium Subsidy program.

If you are currently receiving a subsidy through the IH Assist program, you MUST complete a new application and send updated financial information to be considered for a monthly premium discount for 2013. Your enrollment is NOT guaranteed for 2013 and you MUST reapply for consideration.

The Inclusive Health Assist program is made possible by limited Federal grant funding. As a result, the program may be discontinued at any time. Participation is limited. You are not guaranteed acceptance into the program by applying. IH Assist recipients will be notified of their acceptance into the program.
